Nurse Practitioner Community Health Equity Fellowship

Empowering Family Nurse Practitioners to become leaders in community-based, integrated care

The Strong Children Wellness (SCW) Community Health Equity Fellowship is a one-year training program for Family Nurse Practitioners (FNPs) who are committed to health justice and serving under-resourced communities. Fellows work in our integrated, team-based primary care practice and receive mentorship from SCW’s leadership team who are experts in health equity, systems transformation, and community-based care.

Fellows care for medically and psychosocially complex children and adults, build trust and continuity with their patients, and learn to lead community-engaged health equity projects in partnership with trusted local organizations. Designed to support your growth as both a clinician and systems change leader, this fellowship equips you to transform care delivery for underserved populations.

About the Fellowship

Program Framework

Holistic, Family-Centered Care

Fellows provide care that integrates medical, behavioral, and social services centered on the needs of the entire family. You'll learn to manage complex care within a team-based model and develop skills to navigate family systems, trauma, and adverse childhood experiences

Community Partnership

Fellows work closely with local organizations and the families they serve to co-create solutions to health challenges. Through this process, you'll develop community engagement strategies that emphasize trust, cultural humility, and shared leadership

Equity in Action

Fellows go beyond the clinic walls to tackle the root causes of health inequities. With support from SCW mentors, you’ll lead a community-engaged project that addresses health related social needs and drives change in your patients and the larger community.

Who Should Apply

We welcome applications from licensed Family Nurse Practitioners who have:

  • A strong commitment to advancing health equity

  • Experience working with psychosocially complex and/or
    low-income populations

  • Interest in community partnership and health systems change

  • Passion for working in under-resourced communities

  • Interest in working within multidisciplinary settings that address physical health, mental health and social needs

*All fellows must commit to a minimum of one additional year of employment at SCW after completion of the fellowship

  • Applicants must be licensed Family Nurse Practitioners (FNPs) who have completed an accredited FNP program within the last two years. Candidates should have a demonstrated commitment to advancing health equity and experience working with underserved populations. We seek fellows who have a strong desire to work collaboratively with community-based organizations and are committed to addressing health disparities. Fellows must commit to working with Strong Children Wellness for at least one year after completion of the fellowship

  • The fellowship is a one-year program. Fellows are expected to dedicate approximately 20% of their time (equivalent to one day per week) to the fellowship activities, including clinical work, project development, mentorship, and leadership training. The remainder of their time will be spent in their usual clinical roles.

  • Fellows will receive mentorship from Strong Children Wellness leadership, including guidance on clinical practice, health equity projects, and professional development. They will also have access to coaching and ongoing support from an interdisciplinary team. Additionally, fellows will collaborate closely with community-based organizations on actionable health equity initiatives.

  • Yes, fellows will be compensated as full-time employees at Strong Children Wellness and receive a competitive package of benefits.

  • By the end of the fellowship, fellows will have developed advanced skills in delivering community-based integrated care, gained hands-on experience in health equity initiatives, and built lasting partnerships with community-based organizations. Fellows will also have strengthened their leadership capabilities and be prepared to advocate for and implement systemic changes in healthcare that promote equity for underserved populations.
